The application itself can be completed and submitted online, but you will need lots of supporting materials available in order to complete the application. You can save the application and come back to finish once you get all the required documentation.
If you are also registering a caregiver, you'll need all the same information for the caregiver.
In addition to your application, your doctor will have to fill out and mail a physician certification form.

To complete the application you will need to be fingerprinted by one of these vendors and submit the receipt along with the livescan consent form. Unfortunately I have found no good way to get physical addresses from this list other than to click on one after another link. :-(
You will need an appointment - make sure to tell them the fingerprints are for a Medical Cannabis Card. Also make sure you print the consent form and take it with you for your appointment. You will need both the consent form and receipt to finish your application. If applying for a caregiver card, both patient and caregiver will need to be fingerprinted.
